I am creating a toolbar with css.  I have chosen all the actions with the same result.  The [Name] never links to anything.

Here is my item def.

When uploading the toolbar module initialling, I got this error after the registering had finished.

Does that have anything to do with it?

If the installation weren't 100% successful, then that may be having an impact on your current issue. Uninstall the module, restart the site, then try reinstalling.
